New Patients Doctor Fee is $100.00 Renewal Doctor Fee is $75.00 You can come in anytime and fill out the paperwork to get set up with a appointment to see the doctor. Due to the new regulations we do not offer same day certifications. You can come in anytime during business hours and fill out the paperwork. You will need a valid I.D., and medical records you have withing the past 5 years or know your family doctor’s information: Name, Phone #, Address, and fax. After you fill out the paperwork we will schedule a appointment for you to meet with the doctor. At the doctor visit you will meet with the doctor and go through a consultation.
